How to make a referral for services:

• If you are over the age of 21 and wish to see if you qualify for services, contact: Kepro at 1-833-525-5784. They may schedule an assessment to determine eligibility of services.
• If you would like to make a referral for a child under the age of twenty-one (21), please contact Care & Comfort directly to schedule an assessment to determine the child's eligibility.
• If you wish to make a Private Pay referral contact a Care & Comfort Staffing Coordinator to establish services for in-home care.
• Care & Comfort accepts referrals from community or agency providers, individuals, parents, physicians, schools, hospitals, and many Insurance companies.

If you need help Monday - Friday from 8:00am - 5:00pm please call the branch office nearest you. If you call after hours, you can either leave a message or page our on-call staff who will take your name and number and have the office contact you the next business day.
